An individual consultation consisting of four sessions guided by a systemic-relational perspective is discussed. The patient's problem was considered within her family system. Firstly,the request for counseling seemed to be inherent to the conflict management in a sentimental relationship, but soon, in the clinical context, serious psychopathological traits emerged as strong impulsiveness, lack of self-control, manipulative tendencies, the presence of untreated mourning as well as ideo-verbal phenomena of uncertain psychotic nature. These aspects tested the clinician in a difficult relationship to manage. How to dialogue with a patient who looks at you in a disturbing way, who speaks to you almost screaming, who does not accept points of view different from his, which agitates spasmodically for most of the session and that, while he speaks to you, turns to God and / or dead parents? The consulting process proved to be very difficult and ended with a drop-out.
